Cody is the founder and lead provider of Ozark Men's Health. A board-certified Family Nurse Practitioner with full practice authority in Arkansas, he was born and raised in Hot Springs and returned home to Northwest Arkansas after years of practicing in California and Arizona.

With more than a decade in healthcare, Cody has worked in emergency medicine and hematology/oncology at UCLA Medical Center and Banner Boswell Medical Center, and most recently spent four and a half years as a nurse practitioner in internal medicine at Mercy in Rogers.

Guided by his Christian faith, Cody is committed to helping men reach their full potential as husbands, fathers, professionals, and leaders. Ozark Men's Health is now open in downtown Bentonville.

You receive the requisition by email, walk in at a time that suits you, and the results come straight back to Cody for review at your visit. Quest bills the practice directly, so there is no separate lab bill. Follow-up labs during membership are ordered the same way. Labs should be collected in the morning (7–10 AM) for accurate testosterone levels.
The Weight Loss Evaluation is $149 and includes your first month if you start therapy, the Low T Evaluation is $179 at the founding member rate, and the Comprehensive Health & Longevity Evaluation is $499 in person or $449 by secure video from anywhere in Arkansas at our launch price through Sunday, November 1 ($799 and $749 starting November 2). Every evaluation includes lab work, with no separate lab bill, and the Low T and Comprehensive evaluations credit toward your first month if you proceed. The TRT Foundational Membership covers your testosterone medication, when prescribed following your evaluation (injectable or topical, direct-shipped by a licensed pharmacy), any other clinically indicated ancillary medications discussed during your evaluation, follow-up labs, virtual follow-up visits, body composition tracking, and direct messaging access to Cody. HCG is available as an add-on for an additional cost, since it's a significantly more expensive medication. An oral testosterone (Kyzatrex) membership is also available at a separate flat monthly rate, reviewed at your visit. Medical Weight Loss is a separate ongoing membership, and the two can be bundled if you're on both. No hidden add-ons. A prescription is required before any medication is dispensed.
Candidacy is based on symptoms combined with lab values. Many men fall within the "normal" lab range but still experience significant symptoms. We evaluate the full picture — total and calculated free testosterone, SHBG, LH, FSH, prolactin, estradiol when symptoms call for it, and your symptom burden — before making any recommendations.
Exogenous testosterone can suppress sperm production. If fertility preservation is a priority, we discuss HCG and other clinically indicated options during your evaluation before starting therapy. This is an important conversation to have upfront, and any medication is prescribed only if appropriate for you.
Most follow-up visits are virtual — by phone or video — for your convenience. In-person visits are scheduled when body composition measurements, vitals, or other hands-on assessments are needed. Cody sees patients in person on Mondays and Wednesdays, 8 AM to 6 PM, and the first Saturday of each month, 8 AM to noon, at The Ledger in Bentonville. Virtual visits are Tuesdays and Thursdays, 8 AM to 6 PM.
