Ozark Health and Wellness, PLLC, doing business as Ozark Men's Health ("Ozark Men's Health," "we," "us," or "our"), operates the website ozarkmenshealth.com (the "Site"). This Privacy Policy explains what information we collect through the Site, how we use it, and the choices you have. It applies to the Site itself — general browsing, contact and newsletter forms, and similar interactions. It does not cover the protected health information we create or receive while providing clinical care, which is governed instead by our Notice of Privacy Practices.
Information We Collect
We collect information you provide directly, such as your name, phone number, email address, and any message you enter when you submit our contact form, request a callback, or sign up for our newsletter. We do not ask for or collect health information through these general Site forms; do not include medical details in a contact form message if you'd prefer to keep that information out of routine business correspondence — save clinical details for your evaluation visit.
We also collect limited technical information automatically when you browse the Site, such as your browser type, device type, approximate location derived from your IP address, and which pages you visit. This helps us understand how the Site is used and keep it running correctly.
How We Use Information
- To respond to your inquiry, schedule a consultation, or otherwise communicate with you about our services.
- To send the newsletter you opted into, and to let you unsubscribe at any time.
- To maintain, secure, and improve the Site.
- To comply with legal obligations and enforce our Terms of Use.
We do not sell your personal information. We do not currently use information submitted through the Site for advertising retargeting; if that ever changes, we will update this Policy before doing so.
How Contact Form Submissions Are Handled
Our contact and newsletter forms are processed through Web3Forms, a third-party form-delivery service, which transmits your submission to our practice email inbox. Web3Forms processes and temporarily retains submissions on its own systems under its own privacy practices, and it is not a HIPAA business associate of this practice. On our side, we do not store form submissions in a separate marketing database beyond what is needed to respond to you and, if you opted in, to send the newsletter.
Please do not put medical details in a website form. If you do include health information, we treat it confidentially — and if you later become a patient, information relevant to your care becomes part of your medical record, where it is governed by our Notice of Privacy Practices rather than by this Policy.
Cookies and Similar Technology
The Site uses minimal, functional cookies necessary for basic operation (for example, remembering that you've already seen a pop-up during your visit), together with the analytics cookies described in the next section. We do not use advertising or retargeting cookies.
Analytics
The Site uses Google Analytics 4, a web analytics service provided by Google. It sets cookies and collects information such as which pages you view, how long you stay, the browser and device you use, an approximate location derived from your IP address, and how you arrived at the Site. We use this to understand which parts of the Site are useful and to improve them. We do not send Google your name, email address, phone number, or any health information, and we do not run analytics on scheduling or patient-portal pages.
Google processes this information as a separate company under its own Privacy Policy. If you would rather not be measured at all, Google publishes a browser opt-out add-on at tools.google.com/dlpage/gaoptout.
Third-Party Links
The Site links to third-party services we work with — including Quest Diagnostics, our online scheduling and patient-portal platform (OptiMantra), and our lab-ordering platform and online supplement dispensary (Fullscript) — and to our social media pages. Those third parties have their own privacy practices, which we encourage you to review; this Policy does not cover their websites or services.
Each page of the Site displays a LegitScript certification seal in the footer. The seal image is loaded directly from LegitScript's servers (static.legitscript.com), so LegitScript may receive standard technical information from your browser, such as your IP address and browser type, when a page loads. Clicking the seal or the verification link opens LegitScript's website, which we do not control. LegitScript's handling of that information is governed by its own privacy policy.
Data Security
We use reasonable administrative and technical safeguards to protect information submitted through the Site. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
Calls, Texts, and Messaging
Calls and text messages to our main number are handled through Spruce, a HIPAA-compliant communication platform. If you text us, you consent to receive text messages from our practice about your inquiry and your care. Message and data rates may apply, message frequency varies, and you may opt out at any time by replying STOP — opting out of texts does not affect your ability to reach us by phone or email. Once you are an established patient, most ongoing communication moves to our patient portal.
Payment Information
We do not collect payment card information through this website. When you pay for services, your card is handled by a PCI-compliant payment processor; we do not store full card numbers on our own systems.
How Long We Keep Information
We keep website inquiries only as long as needed to respond and to maintain a reasonable business record, and newsletter contact details until you unsubscribe. Medical records, once you become a patient, are kept separately for the period required by Arkansas law and professional standards.
Children's Privacy
Ozark Men's Health is an adult men's health practice. The Site is not directed to children, and we do not knowingly collect personal information from anyone under 18 through the Site.
Your Choices
You may unsubscribe from our newsletter at any time using the link in any newsletter email, or by contacting us directly. You can also decline to submit information through our forms and instead reach us by phone.
Changes to This Policy
We may update this Privacy Policy from time to time. The effective date above reflects the most recent revision. Material changes will be reflected on this page.
